Meet Your Care Team

Lincoln Health and Wellness Center's care team includes Nurse Practitioners, Medical Support Staff and a Patient Advocate. Each clinician hired by Proactive MD has been identified as a cultural fit for your organization. Below are descriptions on their roles:


  • Family Nurse Practitioner: Family Nurse Practitioners are trained in primary care, health coaching, and chronic condition care and are qualified to treat most ailments and provide comprehensive health care for people of all ages. They also have a supervising physician who they can consult on more complex cases, if necessary, and who also will conduct periodic chart reviews.

  • Medical Support Staff: The nurse will greet you when you arrive at the health center, take you back to the exam room, take your vitals, and draw your blood if lab work is required, and provide point of care testing such as flu, strep and COVID-19.

  • Patient Advocate: The Patient Advocate is a licensed social worker who will help patients navigate the complicated healthcare world, provide behavioral health support, social resources and personal guidance.
