During your visit, and after thorough assessment, if you require a prescription for a condition treated in the health and wellness center, Proactive MD providers may do the following based on whether your appointment is in-person or virtual, whether the medication is on the dispensing list (see formulary below), and what is most convenient for you.
In-Person Visit:
Rx Available on Dispensing List: Dispense some pre-packaged medications at time of visit, at no cost (see formulary list below)
Rx Not Available on Dispensing List: Send prescription to local pharmacy or mail order of your choice, Highmark Blue Shield costs apply
Virtual Visit:
Rx Available on Dispensing List: Send prescription to local Sav-Rx participating pharmacy, at no cost (see formulary list and Sav-Rx information below)
Rx Not Available on Dispensing List: Send prescription to local pharmacy or mail order of your choice, Highmark Blue Shield costs apply
A formulary refers to the list of medications that can be dispensed from within the Lincoln Health and Wellness Center. The Lincoln Health and Wellness Center will not have every medication possible as a pharmacy does. It is limited in that it will carry only generic medications for acute illnesses (i.e. antibiotics, steroid creams) and those that help manage chronic conditions (i.e. blood pressure, non-insulin dependent diabetes and cholesterol).
Note: Narcotics or controlled substances are not carried in the Lincoln Health and Wellness Center.
The same medications that are on the dispensing list will be available through the Sav-Rx program.
The medication dispensing list / formulary is subject to change based on utilization, need and cost. To inquire if your medication is kept in stock, please contact Lincoln Health and Wellness Center.
Lincoln Health and Wellness Center will be able to dispense certain medications at the time of your visit. Medications must be prescribed by one of the Proactive MD providers during an appointment.
We cannot fill prescriptions from outside providers like a pharmacy, such as Walgreens can fill. Our formulary consists of the top utilized medications and the medications used most commonly for diagnosis in a primary care office.
However, if you are currently taking a medication that is available on the dispensing list and you wish to fill it through the Lincoln Health and Wellness Center at no cost, you would just need to make an appointment with a Lincoln Health and Wellness Center provider. They would then be able to write the prescription and fill it within the Lincoln Health and Wellness Center or through the Sav-Rx program.
Note: No narcotics or controlled substances will be stocked in the Lincoln Health and Wellness Center
Medications dispensed onsite or through the Sav-Rx program are available at no cost through the Lincoln Health and Wellness Center when you visit the Proactive MD provider at least once every six months.
Because Proactive MD providers dispense medications using their personal medical licenses, they must have an established relationship with any patient who uses their dispensing services. This ensures that they are prescribing the appropriate medications at the appropriate doses. Even if you are seeing an outside provider, you must visit the Proactive MD provider at least once every six months to continue receiving medications through the Lincoln Health and Wellness Center.
Many times, you may have a virtual visit instead of visiting the health center in person - Lincoln Health and Wellness Center offers a convenient solution to still obtain your prescription (if it is on the available dispensing list) at no cost to you.
Your Proactive MD care team will help locate a Sav-Rx participating pharmacy convenient for you when necessary, and will provide you with a Sav-Rx pharmacy card to show the pharmacy when you pick up your prescription. Local participating pharmacies may include:
Local independent pharmacies
Many local grocery stores with pharmacies
WellSpan pharmacies
Many Rite-Aid pharmacies
Pharmacies NOT included are: Walmart, Walgreens, Sam’s Club, and CVS
Here's what to expect:
During your virtual visit, if the Lincoln Health and Wellness Center provider prescribes you a medication that is on the dispensing list, they will ask for your zip code and walk-through Sav-Rx participating pharmacies available to determine which location is the most convenient for you.
Note: If you are also prescribed medications that are not on the dispensing list, but you would like to pick those medications up at the same time as your Sav-Rx prescriptions, the pharmacy will likely be able to fill those medications as well with the use of your insurance card (Highmark Blue Shield costs will apply).
The Lincoln Health and Wellness Center provider will then electronically send the prescription to the Sav-Rx participating pharmacy selected.
The Lincoln Health and Wellness Center provider will email you your Sav-Rx pharmacy card (you can print this, or use it electronically on your phone). See image of the sample card for reference.
When you arrive at the Sav-Rx participating pharmacy, simply show them the Sav-Rx pharmacy card (either from your phone, or on paper if you printed it) to get your medications at no cost.

Sample Sav-Rx Pharmacy Card
Below is a sample of the Sav-Rx pharmacy card that will be provided to you by the Lincoln Health and Wellness Center provider.
To ensure your medications are in stock and ready when you arrive, please request refills at least 24 hours in advance on days the center is open. Please allow for additional time for requests submitted after hours, on weekends, or holidays. Using the patient portal is the quickest way to request refills, allowing the care team time to prepare your medication and minimize your wait.
How to Request a Refill in the Proactive MD Patient Portal
By selecting the Prescriptions page from the toolbar in the Patient Portal you can view current/active medications and request prescriptions refills. The page that appears will display a list of all medications, as documented in your clinical chart.
To request a refill:
First, select yourself as the patient (or the family member you are submitting a refill request for)
From the Prescriptions & Renewals – Active Meds list, select the Request Refill checkbox for each medication/prescription you wish to renew.
Click the Select & Continue button
Complete the Refill Request form by entering the following information:
Enter the Quantity for each medication that is being refilled
Note: Quantity should be the number of pills that were provided in the previously filled prescription.
Enter any relevant Comments / Questions for the clinical staff or provider
Select a Pharmacy
Note: This will default to the health center location if originally dispensed onsite, or the outside pharmacy used for the most recent prescription; selecting Other will require you to enter the Name and Phone Number for a pharmacy not listed.
Enter Patient Phone Number
Click the Submit button to process your request
The portal will then direct you to the My Refill Request page which will update showing your latest request(s), along with the details of the request.
If there are no concerns with the refill request, the care team will contact you.
