How to Request an Appointment
Appointments are required.
There are two ways to request an appointment:

Through the Patient Portal:
Note: If you are trying to schedule any of the following, please call (717) 640-5900 instead of using the Patient Portal:
A same-day appointment (within 24 hours)
A virtual (telehealth) appointment
Any type of appointment that is not listed
An appointment with the Patient Advocate
If you wish to receive an immunization or lab test ordered by an outside non-Proactive MD provider, to ensure that the necessary supplies are available at the time of your appointment
When can I expect a response to a voicemail or secure message requesting an appointment?
If you leave a voicemail or send a secure message before the health and wellness center closes, you will receive a response the same business day.
If you do so when the health and wellness center is closed, you will receive a response the next business day.
Important Appointment Details
Appointments are required. Please schedule an appointment prior to arriving at Lincoln Health and Wellness Center locations. This helps ensure you and fellow patients have the shortest wait times and best experience possible.
Masks are required. Lincoln Health and Wellness Center and Proactive MD require anyone who enters the health and wellness centers to wear a mask due to the ongoing COVID-19 pandemic.
Cancellations. If you are unable to make your appointment time, please cancel or reschedule as far in advance as possible, out of courtesy for other patients and the Lincoln Health and Wellness Center care team.
Collaboration with outside providers. If you have an outside provider and would like to share your visit information with them, bring the name and contact information of your Primary Care Provider to your appointment, and the care team can send a full summary with your authorization.
After hours healthcare needs. For care needs after Lincoln Health and Wellness Center operating hours, call the main number to be connected to an on-call provider.

All appointments are scheduled for 30-60 minutes, depending on the type of appointment, to allow the provider adequate time to listen to the patient, understanding their unique needs. Longer appointment times not only provide more opportunities to impact and improve long-term health; they also help to build relationships and establish trust between patients and their providers.
Below are average appointment duration by visit type:
Blood pressure checks, lab draw only: 15 minutes
Episodic / acute appointments: 30 minutes
Preventive care / chronic condition management visit: 30 to 45 minutes
Annual physical or establishing care visit: 30 to 60 minutes
