How to Request an Appointment
Appointments are required.
There are two ways to request an appointment:
By Phone: (717) 640-5900
Through Patient Portal Messaging:
Once you’ve been connected to the Proactive MD Patient Portal for the first time during your first visit, appointments can be requested through the messaging feature:
Sign in using your username and password
Select Compose a Message from the top menu bar
Select Message Type: Appointments and scheduling
Select Provider*
Select Location*
Add Subject and Message, include brief details on why you would like to be seen*
* How to request First Available or a Virtual Visit through secure message:
For a first available appointment request at either location with either provider:
Select any item in the Provider and/or Location drop downs
Then, in the body of the message indicate that you'd like first available (or that do you not have a preference)
You can also simply call the main number to request a first available appointment
For a virtual care visit request:
If you'd like a first available virtual visit, or have no provider preference:
Select any item in the Provider and/or Location drop downs
Then, in the body of the message indicate that you'd like a first available virtual visit with any provider
If you have a preferred provider:
Select your desired provider in the Provider drop down, and then any item in the Location drop down
Then, in the body of the message indicate that you'd like a virtual visit with that provider
You can also simply call the main number to request a virtual visit
When can I expect a response to a voicemail or secure message requesting an appointment?
If you leave a voicemail or send a secure message requesting an appointment before the health and wellness center closes, you will receive a response the same business day.
If you request an appointment when the health and wellness center is closed, you will receive a response the next business day.
Important Appointment Details
Appointments are required. Please schedule an appointment prior to arriving at Lincoln Health and Wellness Center locations. This helps ensure you and fellow patients have the shortest wait times and best experience possible.
Masks are required. Lincoln Health and Wellness Center and Proactive MD require anyone who enters the health and wellness centers to wear a mask due to the ongoing COVID-19 pandemic.
Cancellations. If you are unable to make your appointment time, please cancel or reschedule as far in advance as possible, out of courtesy for other patients and the Lincoln Health and Wellness Center care team.
Collaboration with outside providers. If you have an outside provider and would like to share your visit information with them, bring the name and contact information of your Primary Care Provider to your appointment, and the care team can send a full summary with your authorization.
After hours healthcare needs. For care needs after Lincoln Health and Wellness Center operating hours, call the main number to be connected to an on-call provider.
First visits. Bring the following to your first appointment:
Your driver’s license
Your Highmark Blue Shield insurance card
Your current prescription list
Name and contact information of Primary Care Provider (optional) - If you have an outside provider and would like to share your visit information with them, the care team can send a full summary with your authorization.

Appointment Durations
All appointments are scheduled for 30-60 minutes, depending on the type of appointment, to allow the provider adequate time to listen to the patient, understanding their unique needs. Longer appointment times not only provide more opportunities to impact and improve long-term health; they also help to build relationships and establish trust between patients and their providers.
Below are average appointment duration by visit type:
Blood pressure checks, lab draw only: 15 minutes
Episodic / acute appointments: 30 minutes
Preventive care / chronic condition management visit: 30 to 45 minutes
Annual physical or establishing care visit: 30 to 60 minutes

Access to an on-call provider is available after health and wellness center hours by calling the main number.
Holiday Closures
Lincoln Health and Wellness Center will be closed the following holidays or observances*:
New Year’s Day
Martin Luther King, Jr. Day
Good Friday
Memorial Day
Juneteenth
Independence Day
Labor Day
Thanksgiving Day
Day after Thanksgiving
Christmas Eve
Christmas Day
*Proactive MD will observe a holiday closure on the Friday before, if it is on a Saturday, or on the Monday after, if it is on a Sunday.
